Management of your personal information
Some questions will be sent to you via the Consultation Québec platform. The MSSS will collect personal information and health and social services information based on the answers you provide.
The information collected will be used to verify that you belong to the group targeted by the exercise, to understand the context of your participation and to obtain your opinion on rehabilitation centres for young people in difficulty.
If we want to understand your point of view better and you authorize us to do so by checking the box, you may be contacted to provide more details. However, if information of concern relating to a child’s security and development is indicated in your answer, you may be contacted even if you have not checked the box.
The information collected will only be communicated to those responsible for the consultation. They are qualified to receive and analyze it as part of their duties. Any information provided by care providers will not be communicated to their managers.
The information collected will be kept secure until March 2026 at the latest. It will then be anonymized, that is, after this date, it will no longer be possible for the MSSS to identify the people who participated in the consultation.
Participation is voluntary
You are free to agree or refuse to participate in the consultation. You may also withdraw your consent to the use or disclosure of the information collected. To do this, write to the email address smpj_demandes@msss.gouv.qc.ca.
However, if you do not provide the information identified as “required”, you will not be able to participate in the consultation. If you withdraw your consent to the use or disclosure of the information collected, your opinion will not be taken into account in the process of reflection on rehabilitation services for young people in difficulty in Quebec. You will be able to submit a withdrawal request until March 2026. After this date, the information will be anonymized and the MSSS will no longer be able to identify you.
Right of access and correction
You may submit an access to information or correction request as provided for by law by contacting the person in charge of access to information at the Ministère de la Santé et des Services sociaux, Josée Martel, whose contact information is provided below.
Restricted access
It is also possible to restrict access to the information collected to a particular care provider or to a care provider who belongs to a category of care providers, as well as to refuse to allow your information to be accessed by certain people.
